The driver of a tractor which collided with a miniature steam train at level crossing in Dymchurch in September failed to follow instructions posted on signs at the crossing.
That's according to a report published today, external by the Rail Accident Investigation Branch.
The Romney, Hythe and Dymchurch Railway train was carrying up to 80 passengers when the engine and first carriage came off the tracks on 10 September. No one was seriously injured.
The tractor driver hadn't called the railway operators to check he had time to cross, as instructed by signs nearby.
He did stop to let the train pass but judged his position incorrectly and left the tractor partly blocking the line.
Investigators say railway operators should make sure they give proper briefings to those authorised to use crossings - and to consider taking action to reduce potential risks when they're being heavily used.

Kent Police has found cannabis worth more than £20,000 in the back of a car in Margate.
The drugs were found after police asked a driver to pull over for not wearing a seatbelt in Margate on Saturday 26 November, however the driver failed to stop.
The car was found abandoned in Nash Lane with the drugs stashed in the boot.

The Rail Minister Paul Maynard says he's "extremely disappointed" with the announcement that members of the train drivers' union ASLEF have voted overwhelmingly to strike in a dispute over driver-only trains.

The number of people swimming at least once a week has fallen in the past 10 years.
Despite the focus on sport given by London hosting the Olympic Games in 2012, according to figures compiled by Sport England, in Kent there's been a drop of just under 30% and in Sussex and Surrey there have been falls of nearly 19% and 23% respectively.
However swimming remains the most popular participation sport in the country. Read the full story here.

David Thompson, 80, from Snargate in Kent, has been fined £7,500 for killing and injuring swans on his land.
He was sentenced today for intentionally killing and/or injuring mute swans.
The farmer was also ordered to pay court costs of £470.
Mr Thompson was spotted running through a field of rape seed and striking the birds with his shepherd's crook by a coastguard helicopter crew who notified the police. The prosecution was brought by the RSPCA.
