Zoe Zaremba, from Aiskew, who has autism, was last seen at her home at 23:00 on Saturday 13 June.
A major search, involving a police helicopter, dog units and mountain rescue teams, has failed to locate her.
Insp Sarah Sanderson, from North Yorkshire Police, said: "We are very concerned. It is very unusual for Zoe to go missing for such a long period of time.
"She has no means of contact, no communication methods, no money or transport."
Miss Zaremba was last seen by her mother, who she lives with, on Saturday and was reported as missing on Sunday.
She is described as about 5ft 6ins tall, of slim build, with brown eyes and brown hair which she usually wears in a ponytail.
She is believed to be wearing a blue "Keep Calm and Stim On" t-shirt, purple jogging bottoms, white and purple Lonsdale trainers and a silver heart-shaped necklace with an orange crystal.

Two men have been arrested as part of an investigation into "linked" attacks in Bradford this week, police say.
Officers were called to Whitby Road at about 20:15 yesterday to reports of a group of armed men damaging a car with weapons.
When armed officers arrived at the scene, they found a Vauxhall Astra with its windows smashed.
Police believe this incident is linked to two others in the city on Monday night which saw a house and car damaged on Agar Street and a car set alight on Round Thorn Place.
Two men have been arrested in connection with the investigation and have been bailed, according to West Yorkshire Police.
Det Ch Insp Andy Farrell said: "These are being treated as targeted attacks and there is not believed to be a wider risk to the community at this time."
Anyone with information about these attacks is being asked to get in touch with police.
Rachel Ross injured her ankle in a climbing accident and had to face not being able to walk properly again.

Tilly, from Mosborough, wanted to raise money for the eye department at Sheffield Children's Hospital as a way of saying thank you for her care.
She's been looked after by staff at the hospital since October after she woke up one morning unable to see.
Her mum, Natalie Wild, said: "Tilly woke up one day screaming, 'Mummy my eyes have stopped working, I can’t see you.' She also said the light was hurting her eyes."
After an emergency trip to the hospital, the team there noticed she had cataracts on both her eyes.
Ms Wild added: "Tilly is now 10 weeks post-surgery and she’s doing fantastically well, getting used to seeing the world again through her new glasses.
"She’ll soon move onto some special bifocals to help her to start to read."
As a way of thanking the team for their work, Tilly set herself a 30-day scooter challenge, raising £2,255 in the process.
Police have released this CCTV image of a man they want to speak to after a 13-year-old boy was attacked and robbed in Barnsley:
The suspect pushed the teenager to the floor and stole his bike at the junction of Mottram Street and Eldon Street on 15 March after the boy refused to hand it over.
The 13-year-old wasn't injured in the assault and the suspect fled down Mottram Street towards Harborough Hill, police say.
Anyone who can identify the person in the picture, or who saw the attack take place, is being asked to contact South Yorkshire Police.
Airline KLM has said it will restart its route from Leeds Bradford Airport (LBA) to Amsterdam next month having been suspended due to the coronavirus pandemic.
From 4 July, a daily service from LBA to Amsterdam Schiphol Airport will restart to help with essential travel and repatriation, the company said.
Many flights across the world were stopped in March as countries tried to stop the spread of the virus.
Passengers are being told to wear masks during flights and observe social distancing inside airports.
On the plane, KLM says because of low passenger numbers they will be able to separate customers "in most circumstances".

Read MoreDog owners in North Yorkshire are being urged to "take responsibility" for their animals after a lamb was fatally injured in an attack near Boroughbridge.
It happened at about 17:30 on 10 June when two dogs ran through a fence into a field of sheep at Marton-cum-Grafton, near the village primary school.
The sheep in the field had all lambed and one of the lambs was attacked and killed, police say.
Insp Matt Hagen, from North Yorkshire Police’s Rural Taskforce, said: “It’s horrendous for farmers to come across the bodies of sheep who have been attacked and often left to suffer a slow and painful death, not to mention the impact on their livelihoods.
"We need dog owners to take responsibility for their animals. It’s important dogs are kept securely when at home, and on leads and under control when walking near sheep fields."

A five-year-old schoolboy from North Yorkshire has hand-reared a baby bird while shielding during the coronavirus outbreak.
Dylan, from Whitley and Eggborough Community Primary School, has been looking after Jack the starling for five weeks after the week-old bird flew into his sister's bedroom.
Animal-lover Dylan stepped in to help feed Jack.
His mum Sarah said: “It’s been really good for Dylan during the pandemic.
“He has respiratory problems, so has been shielding since March – he’s only left the house a couple of times since then.
“Raising Jack, along with learning about him, has really helped focus his mind on something and distracted him from the situation.”
She said that when Jack was big enough, they released him into the garden, but, after staying around the area for a while, he has since spread his wings.
People considering visiting A&E in Hull due to illness or injury are being asked to think again - unless they're critically or seriously ill.
The request comes as A&E admissions in Hull have increased while staff are still trying to help seriously ill people and those with coronavirus symptoms.
Recently, about 330 people per day have attended Hull Royal Infirmary's A&E - almost double the number visiting when the coronavirus pandemic was at its peak, according to Hull University Teaching Hospitals NHS Trust.
People have been showing up with minor illnesses such as verrucas or minor skin complaints, for which advice or treatment could have been sought elsewhere, the trust said.
Teresa Cope, the trust's chief operating officer, says: "As public restrictions have begun to ease, what we have seen over the last two to three weeks is quite a significant rise in the number of people walking in to the emergency department again.
"We’re asking for the public to really help their NHS once again. This time, we need people to look for services other than A&E if their problem is anything other than a serious injury or critical illness."
People living in Leeds could see council services cut because of a shortfall in funding due to the coronavirus pandemic, the authority has warned.
The crisis has cost the council nearly £200m and threatens "big cuts" to services, it says.
Releasing a series of reports, Leeds City Council said it was facing a shortfall of about £150m, with cash from the government not enough to cover costs associated with the virus.
Council leader Judith Blake said: "The reports show the massive impact coronavirus has had on our city, something which no-one could plan for and which has left us facing a financial challenge of unprecedented proportions.
"Without urgent government support, there is a very real threat of big cuts to council services that many people take for granted."
The authority is now calling on the government for more money in what it says it a "very serious" situation.
