A prisoner found hanged in his cell at the Glen Parva young offenders' institution should have been monitored as a suicide risk, according to a report.

Despite telling drug and alcohol staff and a mental health nurse he had suicidal thoughts, procedures to monitor him had been stopped in July last year. The following month the 24-year-old was found dead.

The prison has now closed for redevelopment. A government spokesperson said they had acted on the recommendations.

HMP Glen Parva accepted and addressed the recommendations made by the Prison and Probation Ombudsman prior to its closure in June 2017. The safety and welfare of people within our custody is our top priority. We are putting more funding into prison safety and are implementing a range of initiatives to reduce levels of self-inflicted death, self-harm and violence across all establishments.”
