A man was mistakenly circumcised in a mix-up at a Leicester hospital, according to a report.

The patient was meant to be having a cystoscopy, a procedure to look inside the bladder using a thin camera, Leicester City Clinical Commissioning Group (LCCCG) has revealed in a document.

It was one of eight mistakes, described as "never events" at University Hospitals of Leicester NHS Trust centres during 2018, the LCCCG said.

Others included a swab being left in a child after a surgical procedure and the wrong patient having an operation after a mix up with their notes.

The trust say never events are "serious, largely preventable safety incidents that should not occur if the available preventative measures are implemented".

The events are reported to hospitals' trust's board when they occur. When asked how the circumcision was allowed to happen, the trust did not provide an answer.

Moira Durbridge, director of safety and risk at Leicester's Hospitals said: "We remain deeply and genuinely sorry to those patients involved, and of course we have personally apologised to each one.

"We are committed to learning and improving and have enshrined this work into our clinical priorities within our Quality Strategy for 2019/20."

    East Midlands Today

Pupils in Derbyshire look likely to keep their six-week school holiday - despite some other areas moving to a shorter summer break.

Headteachers are opposed to a five-week holiday, which is being adopted in Nottinghamshire.

The county council is now also recommending a two week break over Easter, rather than fixing the holiday in April.
