    BBC presenter Jeremy Vine was shocked to find that he could have been charged £990 for a train ticket from Exeter to Paddington.

    It appears that it was actually a mistake and GWR thanked Jeremy in a tweeted reply, external for "flagging this up".

    "Something has clearly gone wrong and we'll make sure its looked into straight away."

    A young offenders institution was found to be "not safe for either staff or boys" after an unannounced inspection.

    The HM Inspectorate of Prisons (HMIP) gave YOI Feltham in west London its lowest judgement for safety after finding levels of "very serious" violence had risen.

    The prison on the same site was also found to have had a "significant increase in violence".

    Cladding fitted to Grenfell Tower during its refurbishment was changed to a cheaper version, documents obtained by the BBC suggest.

    Documents show the aluminium cladding was less fire-resistant than a zinc alternative, saving nearly £300,000.

    The cladding - fitted as part of the refurbishment of the west London tower - is thought to have contributed to the fire that killed at least 80 people.

    The parents of terminally-ill baby Charlie Gard have said his life support machine will be turned off later.

    Chris Gard and Connie Yates have branded doctors "heartless" after their request to bring their son home to die was denied.

    Great Ormond Street Hospital - who believe Charlie has no chance of survival - have refused to comment.

    On Tuesday, Charlie's parents lost their final legal appeal to take him to the US for treatment.
