A care home which didn't regularly give out medicine to residents or have enough staff on duty has been rated as "requiring improvement" in four out of five categories.
The Care Quality Commission (CQC), external said Southfields House care home, which specialises in dealing with people with Alzheimer's, was "not always" safe, effective, responsive or well-led.
In a report, external published on Thursday, the CQC criticised the home - based on Farmhill Road, Southfields in Northampton - as "people did not always receive their medicines from competent and skilled staff".
It added residents were rushed to take medication, were not given it regularly and that medication was not always available.

A spokesman for Northamptonshire Adult Social Services, which incorporates Olympus Care Services, said: "While we are disappointed at the overall rating, we are very proud our staff received 'good' for their care and support of customers.
"As a company we are addressing the issues raised regarding Quality Assurance, and plans are already in place."

Lewis Hamilton and Mercedes claimed a stunning victory over Ferrari's Sebastian Vettel in the Spanish Grand Prix yesterday.
A clever strategic move by the Brackley-based team followed by Hamilton attacking and passing Vettel put the Stevenage driver in control and he defended successfully to the end.
Vettel had passed Hamilton off the start line to lead for the first half of the race but ended up out-flanked by rivals.
Hamilton's second win of the season cut his deficit to Vettel in the championship to six points after five of 20 races.

A 22-year-old man is set to appear at Northampton Magistrates' Court after a man was stabbed and another assaulted outside the BBC office in the town on Saturday.
The area at the top of Abington Street was closed off to pedestrians throughout the morning after the attack at about 06:15. Neither victim received serious injuries.
The alleged attacker was charged with wounding and possessing an offensive weapon and is due to attend court later.
Police are appealing for any witnesses to come forward.
Northampton Saints will have another chance to play in European rugby's top competition next season after Stade Francais beat Gloucester in the final of the European Challenge Cup at Murrayfield on Friday.
The 20th and final place for the 2017-18 Champions Cup competition is decided by a series of play-off matches, and Saints - who finished 7th in the Premiership - would have been replaced by Gloucester had they won the Challenge Cup.
Instead, Saints will now face a one-legged play-off semi-final against Irish club Connacht on Saturday, with the winner progressing to a play-off final against either Stade Francais or Cardiff Blues on Friday, 26 May.

The virus that hit the NHS in England and Scotland, known as Wanna Decryptor or WannaCry, has infected 200,000 machines in 150 countries since Friday.

Kettering General Hospital was hit by the ransomware virus on Friday and Northampton General shut down its systems as a precaution, although both are hoping the full network will be operational later today.
