    Amy Woodfield

A woman from Hinckley says she has received 15 fines for £100 after she checked the wrong box on her prescription form.

Kareena Lees, who is autistic and has several other mental and physical conditions, is entitled to get her medication for free because she receives Universal Credit.

    Prescription form

She said the problem was that officials haven't yet changed the design of the NHS prescription forms to include Universal Credit so she deliberately ticked an incorrect box.

Ms Lees said if she were to pay the fines she wouldn't have enough money for food. Her support worker said this has caused her a lot of distress and anxiety.

The Department of Health told the BBC it was aware of the issues with the current forms and a new prescription form is currently being designed to include a tick box for Universal Credit and will be rolled out in due course.
