    Health Secretary Jeremy Hunt has told MPs he is "profoundly shocked" that one of England's biggest health trusts failed to investigate the deaths of more than 1,000 patients.

    An inquiry - the draft report of which has been seen by the BBC - found there was a "lack of leadership" at Southern Health NHS Trust, where the deaths of mental health patients and people with learning disabilities were rarely investigated.

From next June we will publish independently assured Ofsted-style ratings of the quality of care offered to people with learning disabilities.

    The Varsity Rugby matches take place at Twickenham today with history set to be made for both the men's and women's teams.

    The Dark Blues men can break the record for successive victories with a win today - which would be their sixth in a row since 2009.

    For the women's team their clash is staged at the home of English rugby for the very first time.

  3. Oxford station will be 'a landmark building'published at 10:47

    Arevamped railway stationwould be a "landmark building" for the city, according to county council leader Ian Hudspeth.

    He said the estimated £125million for the project would come from Network Rail, government funding and commercial partners.

    The public will have a chance to give their thoughts on six proposed designs, and Mr Hudspeth said they wanted to "say something about what Oxford is".

    Piper was one of the most significant British artists of the last century and become famous for his stained glass windows, including the South Window at St Mary's Church in Iffley.

    The 36 pieces, mainly paintings, will be auctioned at Mallams.  

  5. Southern Health public inquiry 'should be considered'published at 09:46

    A public inquiry into Southern Health NHS should be considered following a critical report into the way it investigated unexpected deaths, according to the former care minister Norman Lamb MP.

    Southern Health says it has serious concerns about the draft report's interpretation of the evidence, but it accepts its reporting processes following a patient death have not always been good enough.

    Wales and British & Irish Lions centre Jamie Roberts will become the first current international since 2009 to play in the Oxford v Cambridge Varsity match at Twickenham, when he plays for the Light Blues later today.

    The Sue Ryder hospice at Nettlebed, external is holding a recruitment day today to give would-be volunteers a chance to see what kind of work they can get involved with.

  8. Southern Health bosses 'should go', says Connor Sparrowhawk motherpublished at 08:42

    The bosses of Southern Health NHS Foundation Trust should resign after a damning report into uninvestigated deaths, the mother of Connor Sparrowhawk says.

    Sara Ryan's son Connor died at a Southern Health centre in Oxford in 2013, and the report into his death sparked the review of unexpected deaths across Southern Health.

    She says: "The board and the senior team are completely responsible for this... they should have gone already."

    Richard Hudson: "Need something that reflects the history of both the railways and Oxford its self, and not some ugly great edifice to immortalise some public servant."

    Tim Buckell: "Just go down to the Oxford university museum of natural history, take some photos inside and out then copy that down at the station! People will visit Oxford just to see the station."
