    Ahead of today's report into scandal-hit Southern Health, the BBC uncovered new failings at the mental health trust.

    The BBC learned 12 patients have jumped off the roof of a hospital run by Southern Health, while another has escaped twice and is currently missing.

    Problems at Melbury Lodge, in Hampshire, which treats people with severe mental health problems, have been known for years but it is only in recent weeks - following a Care Quality Commission (CQC) inspection - that remedial action has been taken.

    A genetic therapy developed by Oxford scientists has improved the vision of patients who would otherwise have gone blind from the condition choroideremia, external.

    The clinical study has shown that the improvement is long-lasting and so the therapy is suitable to be offered as a treatment.

    The researchers will apply for approval to begin trials to treat more common forms of blindness next year.

  5. Chairman of Southern Health trust quits ahead of CQC reportpublished at 09:20 British Summer Time 29 April 2016

    Ahead of today's damning CQC report into Southern Health NHS trust, the chairman of the trust resigned.

    Mike Petter quit his post yesterday, describing it as a "very difficult decision".

  6. Anger over Didcot A explosives planpublished at 09:01 British Summer Time 29 April 2016

    A family member of one of the workers buried under the rubble at Didcot power station says using explosives bring down the rest of the partially collapsed boiler house is not necessary.

    On Monday, RWE Npower announced its plan to use controlled explosives at the site and said it knew the proposals would be distressing for the missing workers' families.

    Steve Hall, the son-in-law of missing Ken Cresswell, has now asked how "anyone with a heart or a soul could consider an idea like that".

    The trust failed to adopt safe bathing guidelines for two-and-a-half years after Connor died following an epileptic seizure in 2013.

    His unsupervised death led to a report into hundreds of unexplained deaths.
