Mum made 999 call after partner attacked toddler
This was the 999 call from Lola James's mother Sinead after her partner inflicted "catastrophic" brain injuries on the two-year-old.
A child practice review into Lola's death in July 2020, published on Thursday, found "missed opportunities" to protect her from Kyle Bevan at her home in Haverfordwest, Pembrokeshire.
It pointed to a range of problems at multiple agencies, with processes not being followed and an "overstretched" and "low morale" children's services team.
Bevan, 32, of Haverfordwest, was given a life sentence for Lola's murder while James, 30, got six years in jail for causing or allowing her daughter's death.
Pembrokeshire council said it took its safeguarding duties "extremely seriously".
