The Make a Difference Awards are a way to say thank you to people making a positive impact in their communities and some exceptional children have been celebrated with the Young Hero Award.

He's a huge fan of the Wolverhampton Wanderers and started doing matchday summaries for visually impaired supporters around 18 months ago.

Newsround went to meet Frazer just before his big win.
