Louis Sibley scored a first-half winner against West Bromwich Albion to send League One side Derby County into the Carabao Cup third round.
The midfielder curled home for a rebuilding Rams side that was facing Championship opposition for the first time since being relegated last season.
Joe Wildsmith made a number of saves for Derby, with the most crucial being being against Callum Robinson early on.
Both sides had further second-half chances, but Derby held on to advance.
The two clubs made extensive changes following the weekend's action, with David McGoldrick handed his first start for a much-changed Derby side, while Dara O'Shea was the only Baggie to remain in the starting XI after Saturday's thrilling 5-2 win against Hull City.
Jake Livermore took the captain's armband from O'Shea for the tie after coming off the bench during the weekend win.
Rookie manager Liam Rosenior, up against his former Hull City boss Steve Bruce in West Brom's dugout, celebrated another significant win as interim boss of a side that was only taken out of administration last month after being relegated from the second tier while in financial turmoil last season.
West Brom - 14th in the Championship table with just one win from five league games - were made to rue a number of missed opportunities - starting with a shot from Robinson from 12 yards in the second minute, which Wildsmith kept out with his legs.
Sibley's excellent finish after linking up with Lewis Dobbin gave the Rams a 15th-minute lead, and it took a solid save from Alex Palmer to deny the midfielder and the Rams a second just before the break.
Matt Phillips, who had a header kept out by Wildsmith before the break, blazed an inviting second-half chance over the bar after the goalkeeper had parried Robinson's shot.
In a tense finish Sibley had a low shot saved by Palmer, while substitute Tom Fellows lifted a late chance of an equaliser over the bar.
