    Derby will reach the play-offs:

    • If they win
    • If they draw and Middlesbrough fail to win, plus Bristol City fail to win by eight or more goals
    • If they lose by any margin and Middlesbrough lose, plus Bristol City fail to win
    • If they lose by fewer than seven goals and Middlesbrough draw, while Bristol City fail to win

    Middlesbrough will reach the play-offs:

    • If they win and Derby fail to win
    • If they draw and Derby lose by seven goals or more, plus Bristol City fail to win

    Bristol City will reach the play-offs:

    • If they win and Derby lose, plus Middlesbrough fail to win

    Lee Johnson is the first Bristol City boss to be in charge of the club on the final day of four consecutive league seasons since his father Gary did so between 2006 and 2009.

    And if Johnson junior could guide the Robins into the Championship's top six, he'd also be the first to do so since his dad in 2008.

    Bristol City came from behind to win at Millwall on Tuesday to keep their top-six dream alive, having lost at home to Derby last Saturday.

    "It's good for the football club, that we've taken it to the last day," Robins head coach Lee Johnson told BBC Radio Bristol.

    "That element of frustration is there as well. We are very close and some things haven't gone our way, in terms of decisions that have gone for or against us, or last-minute goals that were our own fault.

    "We've got to do our job. We've got to give ourselves every chance, for the other teams to slip up."

    Fans of all three play-off-chasing clubs could well be glued to their phones on Sunday afternoon, but Boro boss Tony Pulis says he will not be trying to find out the scores in the other games.

    "Talking about worrying about what West Brom, Derby, Bristol City and Hull are going to do is irrelevant," Pulis told BBC Tees. "Let's play really well, hope to God that we win, and then look at it then.

    "I'll be really focused on the game and then you get that excitement afterwards or you get that disappointment. I'd rather it that way, myself. With everything going on, I just concentrate on one thing."

    Southend forward Stephen Humphrys scored a late winner against Sunderland to keep the Shrimpers in League One on a drama-packed final day in the third tier.

    The sub, who was wearing a protective mask after breaking his nose, cheekbone and eye socket in February, slotted home with five minutes to go to consign Plymouth to relegation.

    There was controversy in the game between the Pilgrims and Scunthorpe when Iron forward Josh Morris chipped the ball over injured home goalkeeper Matt Macey to make it 2-2 when the keeper had tried to throw the ball out of play to get treatment.

    Scunthorpe chairman Peter Swann apologised for his side's "unsporting" goal in a game where both teams were relegated, despite Plymouth scoring a winner.

    Walsall's 12-year stay in League One also came to an end after they drew at Shrewsbury, though other results meant even a win wouldn't have kept the Saddlers up.

    Elsewhere, Luton sealed the title with a 3-1 win over Oxford and Doncaster took the last play-off spot with a 2-0 success over Coventry.

    Notts County were relegated from the Football League for the first time in their 157-year history.

    County needed to win at Swindon and hope that Macclesfield lost at home to Cambridge in order to retain their status as the Football League's oldest club but they fell to a 3-1 defeat while the Silkmen held out for a 1-1 draw.

    Elsewhere, MK Dons secured an immediate return to League One by beating promotion rivals Mansfield.

    The Stags will now face Newport in the play-offs after they finished seventh thanks to a late Jamille Matt equaliser at Morecambe.

    Derby boss Frank Lampard says reaching the play-offs would be one of his biggest achievements, if his side can secure sixth spot today.

    "It would be an incredible achievement for the players to get there this year, with the competition around us," the former England midfielder said. "I know the players will give everything. It's a great opportunity.

    "I know this is the play-offs, so I don't want this to sound like I'm over the top, but it would be one of my biggest achievements, because it's in my first year."

    With their play-off spot confirmed Aston Villa make five changes to the team that drew with Leeds last week, with full-back Alan Hutton coming in to skipper the side for what could well be his final appearance for the club. Norwich City boss Daniel Farke sticks with the same team that secured promotion with a 2-1 win over Blackburn last week.

    Stoke City boss Nathan Jones includes departing midfielder Charlie Adam on the bench, Sheffield United skipper Billy Sharp returns in place of Scott Hogan.

    Relegated Ipswich bring in Andre Dozzell and Collin Quaner up front, while Leeds boss Marcelo Bielsa makes just one change from the draw with Villa, Kemar Roofe comes in for the suspended Patrick Bamford.
