NHS death investigations to be reviewed

A review is to be held into how the NHS investigates deaths following a damning report into a mental health trust.

Southern Health has been strongly criticised for failing to carry out proper investigations into hundreds of unexpected deaths, as the BBC exclusively revealed last week.

Today, as the official report was published, the chief executive of Southern Health apologised unreservedly and said improvements in the way investigations were conducted had to be made.
