NHS 'failing' to respond to patient safety alerts

All seven health boards in Wales are failing to implement vital safety alerts, a report by a leading patient's safety charity says.

Action Against Medical Accidents says alerts are introduced to protect patients when things go wrong, but the NHS in Wales has failed to meet the deadlines.

They do say though that things have improved since last year.
