'Key lessons' for out-of-hours NHS in Wales
The public services ombudsman says there are key lessons for the NHS in Wales to learn about out-of-hours care - and there is no room for complacency.
Nick Bennett wants a systemic review after publishing a snapshot of 12 cases which he said "inadequate standards of care" were given to patients, including at weekends.
The Welsh government said it would look carefully at the report but said quality was at heart of the NHS.
Mr Bennett told BBC Wales health correspondent Owain Clarke it was important the NHS listened to what patients were telling him.
