Hajj stampede a 'disaster waiting to happen'

A deadly stampede near the Islamic holy city of Mecca in Saudi Arabia was a "disaster waiting to happen", according to the BBC World Service's Middle East editor.

Sebastian Usher says while there has not been a major incident at the Hajj for nine years, the sheer numbers of people involved in the Muslim pilgrimage means this may have been a "disaster waiting to happen".
