Liverpool Care Pathway replaced by patient-focused guide

Doctors and nurses are being urged to treat dying patients as individuals rather than making "snap decisions" about their care.

The health watchdog NICE says staff in England need to move away from a "tick-box approach" to treatment at the end of people's lives.

The guidance is intended to address misuse of the previous system, the Liverpool Care Pathway, which left some patients heavily sedated, and without food and water.
