A confidential police report into how a Staffordshire Police unit handled protected witnesses during a murder investigation concluded it needed radical restructuring, external.
Suspected drug dealer Kevin Nunes, 20, from Wolverhampton, was found dead in a country lane in Pattingham, Staffordshire in September 2002.
Staffordshire Police have today publishing a redacted copy of the 2006-7 Management Review of Staffordshire Police's Sensitive Policing Unit.
The publication follows Freedom of Information requests made by the Express and Star newspaper. , external
Five men jailed for his murder were cleared on appeal in March 2012. Two of the men who were cleared have began legal action against Staffordshire Police.
The report details failings in how the unit was run and managed in 2006-7 including a breakdown in trust between officers, concerns over officers drinking alcohol on duty, poor morale and friction between staff members.
Staffordshire Police says the unit was disbanded in 2007 and management of protected witnesses has been done on a national basis since 2012.
Tributes have been paid to Graham Taylor in a meeting between two of his clubs just days after his death.

A third man has been charged in connection with the riot at HMP Birmingham last month.
Inmate Carl Brookes, 32, will appear before magistrates tomorrow accused of prison mutiny and taking a photograph inside the Winson Green site on 16 December.
The privately-run jail was hit by disorder which led to the transfer of hundreds of inmates.
Two other men have been charged over the riot and are due to appear at Birmingham Crown Court next month.

Rail Pastors – an initiative made up of trained volunteers from local churches – will patrol platforms and trains on the line between Stourbridge and Birmingham Snow Hill.
It's a bid to support police officers who last year, British Transport Police said, attended more than 460 mental health incidents, 21 suspected suicides and six people who are believed to have deliberately harmed themselves on the railway.

A 40-year-old West Midlands man has died after a crash on the A361 near Ashby St Ledgers, Northamptonshire, on Saturday.
The incident happened at about 10:20, between Kilsby and Daventry, when the man's car collided with a lorry travelling in the opposite direction.
The man from Dudley, near Birmingham, died at the scene.
Northamptonshire Police has appealed for witnesses.
A burst water main in Tyseley (B11) is affecting the water supply.
Severn Trent Water says , externalit is will "will work around the clock to get this pipe fixed and everything back to normal as quickly as possible".

An off-duty nurse from the Alexandra Hospital in Redditch performed life-saving CPR on a man who'd collapsed and banged his head while out with friends.
The man in his sixties was unconscious and had stopped breathing after he collapsed in Birmingham on Saturday afternoon.
Marie Hulston was out drinking with friends when she saw what was happening and rushed over to help.
Quote MessageI don't like to say I'm a nurse if I'm out socialising and then once I'd seen he was unconscious, I thought, 'I need to sort him out'. I turned him over and obvioiusly he was purple. His friends were hysterical, they was just shouting his name constantly. I had to tell them to get away and then I had to start CPR."

The hearings at London's Royal Courts of Justice are expected to take seven weeks.
Live TV feeds of the proceedings will be shown in courts in Cardiff and Stirling to allow survivors and relatives to follow them.

A BBC investigation has uncovered people selling unwanted or unused drugs, which can only be prescribed by specialists.
Prescription medicines to treat a variety of ailments were readily available for sale on eBay.
An eBay spokesperson confirmed it had removed a number of listings for sale from the site.
