A British camera operator who died while shooting a stunt sequence for a BBC drama in Ghana was from Herefordshire.
Mark Milsome, whose credits include Saving Private Ryan and Sherlock, was working on The Forgiving Earth.
The BBC said it was "deeply shocked and saddened" by the news.
His agent said an investigation was under way.
A man has appeared in court charged with murdering his wife.
Police were called to the home they owned in Knightcote, Warwickshire, on November 15 where they discovered 56-year-old Patricia McIntosh with head injuries.
Andrew McIntosh, 54, of Woolscott, Warwickshire, was arrested later that day.
Appearing via video link at Warwickshire Justice Centre, he was remanded back into custody until a plea hearing in January.
David Clarke, of Nuneaton, faces eight charges in connection with a four-hour police siege.

More than 1,850 people have signed a petition against plans to fine rough sleepers in Stoke-on-Trent.
Last week the council announced plans to fine anyone found sleeping in tents £1,000.
The results of the online petition are due to be handed to the council next month.

A former headteacher has been charged with 15 child sex offences and a charge of child abduction.
Timothy Moule, 46, from Shropshire, has been charged with one count of child abduction, six counts of making indecent images, five counts of voyeurism, two counts of taking indecent images and two counts of causing a child to look at a sexual image.
He was first arrested by detectives following an allegation of child abduction of a boy from the West Midlands.
He is due to appear at Walsall Magistrates Court later.

An honorary doctorate has been awarded posthumously to Delia Derbyshire, the composer who brought to life the Doctor Who theme music.
It was collected on her behalf during a ceremony at Coventry University on Monday night by partner Clive Blackburn and Mark Ayres, composer and sound designer at the BBC Radiophonic Workshop from which the music originated.
Derbyshire was honoured for her contribution to electronic music - the most famous of which is her arrangement of Ron Grainer's Doctor Who score.

A restaurant in Oswestry is fighting to save its Christmas party nights amid claims celebrations will be too noisy.
The Townhouse is planning to put up a teepee in its walled garden and charge £40-a-head for Christmas parties - with food and live music around the tent’s fire pit.
Shropshire Council’s environmental health department has objected in order to protect neighbours from noise, meaning the case will be heard by councillors on the licensing committee later. , external
The Townhouse says it’s had only two complaints in the three years since it opened, and holds music events in the garden at other times of the year without objection under its regular licence.

Some of Coventry's historic buildings are to get fresh funding through a partnership with the city council and the Historic Coventry Trust (HCT).
They're teaming up to try to secure the future of places including Drapers' Hall, Charterhouse and the Burges.
It's hoped the funding can help bring them back into use and make them cultural destinations.
The HCT can access funding the local authority cannot, the council said.
A total of 159 firearms have been handed in to police stations across parts of the West Midlands in the first week of a new initiative.
People in Warwickshire, Herefordshire, Worcestershire and Shropshire are being urged to surrender them at a police station.
Twenty-nine weapons were left at Leamington Spa Police station, and 26 in Telford.
The weapons can be handed in anonymously - but the force will check them later to see whether they've been used in any criminal activity.
