Former Crewe Alexandra manager Dario Gradi has been criticised for failing to pass on allegations of child sex abuse, while he was assistant manager at Chelsea in the 1970s.
A report paid for by Chelsea and led by barrister Charles Geekie QC, said young footballers were targeted for years at Chelsea by former chief scout Eddie Heath, who is described as a "prolific and manipulative sexual abuser".
The report said Chelsea "turned a blind eye to what they saw" and Gradi is accused of failing to tell more senior club staff about an allegation regarding the sexual conduct of Heath, brought to him by the parent of a young player.
Gradi's failure to report the allegation "was a lost opportunity to expose Heath and prevent further abuse", the report said.
In a statement, Chelsea's board apologised "unreservedly" and said Heath's "abuse was able to occur unchallenged".
The BBC has approached Gradi for comment. In his evidence to the review, he denied trying to "smooth over" the matter in a meeting with the boy's father. He also said he reported the allegations to the club's assistant manager.
Gradi was suspended by the Football Association in December 2016.
The singer is recovering at home and "needs space to get better", according to his brother Merrill.

The company carried out an assessment of 33-year-old Victoria Smith from Market Drayton in March 2018.
The assessment led the DWP to decide Ms Smith, who suffered from agoraphobia and fybromialgia, was no longer eligible for PIP (the main disability benefit).
Ms Smith died of a brain haemorrhage in July 2018.
Her mother, Susan Kemlo, took legal action against the company for maladministration and was awarded £10,000.
But the company is now challenging that court ruling on the basis it never had a chance to defend the case, because of problems in its own internal mail system.
Capita said it's assessment was "fit for purpose, accurate and medically justified" and is taking the action to reverse the "reputational damage" it says it suffered after the ruling.
A 36-year-old man has been arrested after a woman was sexually assaulted at Selly Oak station in Birmingham.
The incident happened during the early hours of Friday.
The man remains in police custody, said British Transport Police.

A racially aggravated assault in Nuneaton is being investigated, police say.
The victim, a man in his 30s, was walking with his young daughter in Whittleford Park when he was assaulted by a man who was racially abusive before he made off.
The incident happened at about 10:00 on 2 August on football pitches at the rear of the playground.
The "nasty and unprovoked" attack left the man shaken, said Warwickshire Police.
Any witnesses are being asked to come forward.
