Officers say an axe was held to a man's throat, who was also punched repeatedly at the Palash restaurant on Birmingham New Road on 4 September.
The 19-year-old victim had also been attacked by one of the suspects in a road rage incident outside at about 17:50.
The three men then stormed into the takeaway, police said, before they escaped in a black Renault Clio.
Officers are appealing for any witnesses to come forward.

A former Salvation Army building in Oswestry is going to become a Muslim centre.
Planning consent was granted in December 2017 to turn the Citadel building in King Street into a cinema, but when nothing came of those plans, an application was made to turn it into a centre for Muslim prayers and social gatherings instead.
The person behind the application, Zacharia Chowdhury, said there would be prayers five times a day, but no external calls to prayer.
And he said the change of use would mean the building, which has also previously been a carpet and blinds store, would be "maintained and retained for community and generations of the future”.

The book was originally bought for £10.99 in 1997 as a gift for the anonymous seller's children and Hanson's Auctioneers said it was in "pristine condition".
The auctioneers haven't revealed the name of the buyer, who is from London.
The original guiding price for the book was between £25,000 and £30,000, but prior to the bidding, auctioneer Jim Spencer said he "wouldn't be surprised if it doubled that".
He said the owner decided to sell the book after another first edition was found in a cupboard and sold for £28,500 after being bought for just £1 from Staffordshire libraries.

A man stabbed in Smethwick on Wednesday had tried to intervene in disorder that broke out after a robbery, police said.
The 24-year-old is in hospital with injuries not thought be life-changing.
Officers were called to reports of the fight on Rosebery Road at 15:30, said West Midlands Police.
A man in his 20s was also found with head injuries after being hit with a baton, the force said.
A 15-year-old boy and 24-year-old man were arrested on suspicion of violent disorder and remain in police custody.
