The assessment of Charlotte Robins' mental state was "fundamentally flawed" according to a report from the Parliamentary and Health Service Ombudsman.

She died in 2010 aged 29, weighing less than five stone.

Her parents have being pursuing complaints against the service from Devon Partnership NHS Trust ever since. After six years, the ombudsman has agreed with them, saying a "service failure" led to an "injustice" for the family.

The Devon Partnership NHS Trust has apologised for what it calls shortcomings in the way it originally investigated the incident. It says important changes have been made to its services.

    BBC Spotlight

A Devon father is demanding better treatment for people with eating disorders, after an investigation found his daughter wouldn't have died if aspects of her care had been better.

Charlotte Robins died in 2010 aged 29, weighing the same as a small child.

Her parents always insisted there was a catalogue of errors in Charlotte's treatment by the Devon Partnership NHS Trust, a view upheld by the Parliamentary and Health Service Ombudsman.

Its report was not made public until the family decided to share the findings with the BBC, concerned action was not being taken quickly enough.
