A coroner investigating the death of a 62-year-woman from Sheffield on a stretch of smart motorway is considering whether to refer the case to the Crown Prosecution Service (CPS).
Grandmother Nargis Begum died in September 2018 after the car she was a passenger in broke down near Woodall services in South Yorkshire.
She and husband Mohammed Bashir, 67, who was driving, got out of their vehicle and were waiting for help to arrive when another vehicle collided with theirs, causing it to hit Mrs Begum.
At a pre-inquest review in Doncaster, coroner Nicola Mundy heard how 16 minutes and 21 seconds elapsed between the Nissan breaking down and the collision. And she said it was a further six minutes and 15 seconds before warning signs were activated.
Ms Mundy said this made a total of 22 minutes and 36 seconds between the breakdown and warnings being displayed. The coroner said: "I want to know why, essentially. It's as simple as that."
Ms Mundy said she was considering whether to adjourn the inquest in order to make a referral to the CPS about any potential homicide.
She said this included whether Highways England had a case to answer for either corporate manslaughter or gross negligence manslaughter.
The case was adjourned until 11 February.
Staff say the special visit brought "lots of smiles to those brave little faces".

The Canal and River Trust says there’s no immediate flood risk to properties in East Cowick and is confident the floodwater will be contained.
John Ward from the trust said although the soft earth had washed away letting water through, the solid piles in the canal bank appeared to be holding.
He said: "We've got the piles there, but clearly we need to fill that hole and that's what the plans is.
"Bring the helicopter in, lift those stone bags in. That will fill the hole and then we'll bring the clay in and that will provide the seal to stop the water leaking in."
An appeal for witnesses has been launched following the death of a 17-year-old who was killed when the car he was driving hit a bridge in West Yorkshire, say police.
Emergency services were called to the entry slip road on the A64(M) Leeds at about 21:30 on Saturday, according to West Yorkshire Police.
A police spokesperson said: "A Ford Fiesta left the carriageway and was in collision with a bridge.
"The driver a 17-year-old male was taken to hospital and died from his injuries.
"Two male passengers, aged 17 and 18, were taken to hospital but have since been discharged."
Anyone who saw the collision or who saw the Fiesta prior to the incident, is asked to contact police.
A mass Covid-19 vaccination site will open in York on Tuesday.
The open-air site at Moor Lane car park, Askham Bar, is currently being used as a flu vaccination site.
A primary care provider will deliver the vaccine to eligible patients working with 11 GP practices in the area.
Patients will be invited to attend for vaccination by their surgery and will have to return for a booster jab at a later date.
York-based primary care provider Nimbuscare Ltd said the vaccine would be given to priority patient groups first.
Professor Mike Holmes said: "This programme is one of the most important moments in the history of the NHS.
"Delivering the vaccine on this scale is a huge task and we're fortunate to have a dedicated and experienced team which is making this possible."
No passengers are being allowed on ferries between Hull and Rotterdam and Zeebrugge.
It's after a number of European countries banned travellers from the UK because of a new variant of coronavirus spreading in the south east of England.
P&O Ferries say the routes to and from Hull will be freight only for at least 48 hours.

A 39-year-old man has been arrested after an 18-year-old woman went missing from Halifax.
Tara Whelan was last seen in Market Street in Halifax town centre on the morning of 15 December, according to West Yorkshire Police.
It's thought she was in the Parkinson Lane/Queen's Road area of Halifax at about 13:00 later that day.
Tara is described as a white female, 5ft 7in tall, of medium build, with a pale complexion, blonde hair and glasses.
She was last seen wearing a white top, black leggings, dark green parka coat with fur around the hood, black ankle boots and a large purple over the shoulder handbag.
Det Supt Mark Swift, said: “It is out of character for Tara to be out of contact with her family for this long and we are growing increasing concerned that she has not be in contact with them.
"I'm appealing to anyone who knows where Tara is, or who has seen her over the last few days, to come forward"
The man arrested in connection with Tara's disappearance is currently being held in police custody, officers said.

The 19-year-old man, who has not yet been named, was killed when the silver Ford Fiesta crashed on Hathersage Road in Dore at about 22:15, according to South Yorkshire Police.
A spokesperson for the force said: "It's believed the 17-year-old driver of the car lost control on the bend, causing the car to flip."

An investigation into the death of a 26-year-old man in Huddersfield is continuing following the arrest of a man on suspicion of murder.
Police were called to Ripon Avenue in Fartown, at 22:09 on Saturday after reports of shots being fired.
The 26-year-old victim was found injured and taken to hospital where he died shortly afterwards.
A spokesman for West Yorkshire Police said the arrested man remained in custody and several roads had been closed in the area.
Foot patrols have also been increased and the force appealed for anyone who saw anything suspicious to contact them.
