The family of Jimmy Carol, who's originally from Chapel-en-le-Frith in Derbyshire, say since being admitted to hospital in Alicante for emergency surgery his condition has declined.
They say doctors have prepared them to consider "the worst possible outcome".
Reports claim Mr Carol was punched after confronting a group of men in a bar who were verbally abusing a group of girls.
Mr Carol's family are being assisted by the British Consulate and are working with Spanish Police.
Those involved are still to be identified.
Organisers faced pressure from the fire service and the Peak District National Park.

There are delays in Chaddesden at the moment due to a police incident.
The Derby Telegraph says, external an air ambulance was called after a woman fell off her mobility scooter near the junction of Nottingham Road and Chaddesden Park Road.

Police have released a photo of a man they want to speak to after a woman was assaulted and threatened during a road rage incident in Wessington.
The confrontation took place in Wistanes Green at about 17:00 on 5 June. Anyone who recognises the man is asked to call 101.

The Leicester City striker was a second-half substitute against Colombia on Tuesday and looks a major doubt for the Sweden game due to a groin strain that needed an injection after the match.

A 10-year-old boy has been found living with homeless people after disappearing from a children's home.
It's understood the child went missing from the West View home in Pleasley, Derbyshire, for more than three days last week.
Nottinghamshire County Council has confirmed a "major incident" was declared due to his age.
A police helicopter was brought in to assist search teams on the ground before he was eventually found on Sunday with the homeless group.
The county council said it could not discuss details to protect confidentiality but a spokesman said the boy was found safe and well after a social media appeal by police.
There will now be a formal investigation into the disappearance.
Injured England bowler Stuart Broad expects to return to action for Nottinghamshire's second team in late July.

This misspelt road marking has prompted a mixture of groans and giggles among people in Newark.
The painted warning on Kirk Gate now tells drivers a bay is for "loading olny".
The county council said it had ordered the contractor to redo the markings.
Derek Higton, service director for place and communities, said: “We are very disappointed that the work to renew road markings in Newark, including the double yellow lines, are not up to the standard we would expect.
“The work was carried out by a subcontractor of Tarmac, on behalf of Via East Midlands, who are overseeing the work for the council.
“The sub-contractor has been instructed to urgently redo the work at their own expense. The road markings/double yellow lines in their current state do remain fully enforceable.”

Nottingham City Council has been criticised for the way it treated a mum during a schools appeal hearing.
The mother had asked for her son to be moved to a new school because of "honour-based violence" in the family. The school in question was full, so the case went to appeal.
The Local Government Ombudsman found instead of giving the mum a chance to put her case, she was repeatedly questioned in a confrontational manner, despite having supporting evidence from the police and health practitioners.
The council has been ordered to hold a fresh appeal with a different panel and give its officers extra training.
