    • The A34 is very busy in patches northbound from Hinksey Hill up to the M40 junction
    • In Oxford there's delays on the Cowley Road in both directions because of some temporary traffic lights

    • The trust could not demonstrate a comprehensive systematic approach to learning from deaths
    • Despite the trust having comprehensive data on deaths, it has failed to use it effectively 
    • Too few deaths were investigated in Learning Disabilities and Older Peoples' Mental Health sectors
    • Review highlighted cases which should have been investigated further but which were not and should have been
    • In nearly two thirds of investigations, there was no family involvement
  9. Connor Sparrowhawk mother 'sickened' by Southern Health reportpublished at 17:25

    The mother of Connor Sparrowhawk, who died in the care of Southern Health NHS Trust, described a report into uninvestigated deaths at its centres as "sickening".

    Sara Ryan says: "We were suspicious because we thought it was really fishy to say that Connor's death was due to natural causes... we were rocked to the core when we read it and I remain so."

    Connor's death in 2013 found to be preventable, and the investigation into his death sparked the wider report into all unexpected deaths in Southern Health centres.

  10. Southern Health deaths investigation varied by patient typepublished at 17:17

    Of the 1,454 unexpected deaths of people in the care of Southern Health between April 2011 and March 2015:

    • 272 were treated as critical incidents of which just 195 - 13% - were treated by the trust as a serious incident requiring investigation
    • The most likely group to see an investigation was adults with mental health problems, where 30% were investigated
    • For those with learning disability the figure was 1%
    • Among over 65s with mental health problems, it was just 0.3%

    Southern Health NHS Foundation Trust says it has "serious concerns" over the draft report which criticises it for not investigating the unexpected deaths of more than 1,000 people.

    A statement from the trust says: "There are serious concerns about the draft report's interpretation of the evidence."

    It adds the trust "fully accept" that its reporting of patient deaths was not good enough and "considerable measures" had been taken to improve investigations of deaths.

  12. Southern Health NHS Trust 'failed to investigate deaths'published at 17:01

    An NHS trust which cares for thousands of people in Oxfordshire has failed to investigate the unexpected deaths of more than 1,000 people, according to a damning draft report obtained by the BBC.

    The report, commissioned by NHS England, says too often Southern Health NHS Foundation Trust missed opportunities for learning.

    The review into all deaths in Southern Health care was sparked by the death of Oxford teenager Connor Sparrowhawk at Slade House in Headington, which was found to be preventable. 

  13. Thousands sign petition to ban Donald Trump from UKpublished at 16:43

    More than 2,500 people in Oxfordshire have signed a petition calling for Donald Trump to be banned from entering the UK, external.

    The petition was launched after the US presidential hopeful said all Muslims should be temporarily barred from entering the US in the wake of the mass shooting in California.  

    Former Oxford city councillor, Fiyaz Mughal, founder of the organisation Faith Matters which tries to reduce inter-faith tension, says Mr Trump's comments are worrying.

  14. Police appeal for witnesses after Summertown robberypublished at 16:28

    A Carphone Warehouse shop in Summertown in Oxford has been robbed in broad daylight, external.

    Three men pulled up in a car before entering the shop, demanding keys to locked cabinets and the back room.

    They filled a bag with electronic equipment and personal items from the staff who they forced into the toilets, before leaving in a large dark-coloured saloon car.

  15. Duke of Marlborough impersonator jailedpublished at 16:07

    A man who posed as the owner of Blenheim Palace, the Duke of Marlborough, and racked up bills of more than £10,000 at luxury hotels has beenjailed for three and a half years.

    Alexander Wood, 34, and from Southend in Essex, pleaded guilty to defrauding luxury hotels.

    He booked into the Great North Hotel under the duke's name, Jamie Spencer, in May this year and set up an agreement where he would be allowed to spend £100 a day on "incidentals" such as meals and taxis, which would be added to the bill.

    There's a danger the £120m flood alleviation channel planned for Oxford will never get built - according to the director of Oxford University's Environmental Change Institute.
