Cannock Chase Clinical Commissioning group says it will now take steps to have the first inquest verdict into Jonnie Meek's death quashed and a second inquest held.
Andrew Donald, Chief Officer for the Staffordshire CCGs, said the complaints procedure had "failed" and that he was "convinced" that if the family had been listened to when the trust received the original complaint "then we would not have had to commission this review".
The death happened when Stafford Hospital was run the Mid Staffordshire NHS Foundation Trust, which has since been dissolved.
Quote MessageWe are pleased that Dr Farrier concluded that medical staff did everything in their power to keep Jonnie alive – there is no suggestion that anyone did anything wrong in the medical care provided.

The independent case review's report has been put together by Dr Martin Farrier, a consultant paediatrician at Wrightington, Wigan and Leigh NHS Foundation.
He concluded:
- Jonnie’s reaction to the new milk feed was a “very unlikely” event which could not have been predicted by the medical staff caring for him.
- It cannot be conclusively stated that his reaction to the milk feed was the cause of Jonnie’s death, but Dr Farrier regards it as the “most plausible” of a range of unlikely events.
- The response of the medical staff to Jonnie’s deterioration was “exceptionally rapid” and a “very experienced team” attempted resuscitation.
- Given the conclusion reached by the original inquest that Jonnie died of “pneumonia natural causes”, a new inquest should be held.
- Mid Staffordshire NHS Trust offered a “poor” response to the family’s concerns and did not respond adequately to them.

The parents of three-year-old Jonnie Meek from Cannock have always disputed a coroner's conclusion that he died of "natural causes".
In August 2014, the toddler visited the former Stafford Hospital for a routine procedure - a genetic condition known as "De Grouchy Syndrome" meant he had to be fed through a tube to his stomach.
While nurses changed the type of milk formula in his system, his health deteriorated and, despite efforts to resuscitate him, he died.
His parents claim the milk caused an allergic reaction, contrary to the inquest conclusion that he died of pnuemonia.

A review into the death of a toddler at Stafford hospital last year has called for a new inquest into how he died.
Three-year-old Jonnie Meek had a rare genetic condition which meant he had to be fed through a tube into his stomach.
He died in August 2014 after nurses attempted to change his feed.
An independent doctor has now called for the conclusion that he died of "natural causes" to be quashed.
