Hospital theatre staff at the Medway Maritime Hospital, in Gillingham, are being balloted for strike action and industrial action short of a strike.
The dispute centres on new rosters introduced earlier this month "without agreement".
The Unite union said in one case it meant shift members working 12 hours without being relieved for their 30 minute unpaid break, with just a 10 minute break for a glass of water between patients.
It is balloting 74 anaesthetic practitioner, staff nurse, obstetric and theatre recovery members, and has warned a vote for industrial action could lead to cancelled and delayed operations in the new year.

One person was arrested on suspicion of several offences, including failing to report a collision, leaving the scene of a collision and failing to provide a breath sample.
Officers want to hear from anyone who saw what happened.
Crowd trouble around the Brighton v Crystal Palace match has been described by a top police officer as the "most challenging football-related disorder" he has ever experienced.
Six people were arrested following the trouble on Tuesday evening, which saw two stewards taken to hospital.
Ch Insp Simon Nelson, who led the police operation, said: "I have been involved in football policing at all levels and have been a match commander for Albion matches for 12 years, but I have never come across such concerted attempts to cause disorder and engage in violent behaviour."
He said groups of masked supporters, from both sides, were seeking confrontation with each other and police around Brighton.
He added that a number of fans attempted to enter the ground with pyrotechnics, knives and knuckledusters, which were found following efforts to gain access through fire exit doors in the south stand.
Out of the six arrested, three people have been charged with a range of football-related offences, including failure to comply with a banning order and using threatening behaviour.
The three others are still under investigation.
The Football Supporters' Federation investigates claims that Crystal Palace fans with tickets were denied entry to Tuesday's game at Brighton.

Guernsey government owned airline Aurigny says it has seen record passenger numbers on its Gatwick route.
Between August and October there were nearly 90,000 passengers flying between Guernsey and Gatwick.
Commercial Director, Malcolm Coupar says it points towards a strong end of year with bookings already up for November and December.
The news comes as Aurigny plans to launch its own dedicated team at Gatwick Airport with the training of 20 ground handling operators recently completed.
Mr Coupar says it is part of a move to give customers the best possible service on the island's lifeline link.
The junior and senior citizens get together three times a week.

New warning signs have been installed at a level crossing following four near misses at the site since May.
The Hatches level crossing, between Frimley Green, in Surrey, and Farnborough North railway station, in Hampshire, is the second highest risk footpath crossing on the South Western Railway network.
Users have to decide for themselves whether it is safe to cross.
Level crossing manager Kathy Welch urged people, specifically school children, to take off their headphones or put their phones in their pockets.

Reverend Herbert Butler Cowl was the only army chaplain to be awarded the Military Cross for bravery.
He was honoured for his actions on a ship that sank off the coast of Kent during World War One.
His granddaughter has told his story in a book, called The Half-Shilling Curate.

Over 300 firearms, including two sub-machine guns and two assault rifles, were handed in to Kent Police during a firearms amnesty.
The force also collected 32 rifles, 49 revolvers, 46 pistols, 73 shotguns, 61 air rifles, 36 air pistols, five gas guns and nearly 10,000 rounds of blank and live ammunition.
The amnesty ended on Sunday.
