The biggest shock of the night was on the cards when AFC Wimbledon took the lead against West Ham, but Manuel Pellegrini's men recovered to beat the 10 men 3-1.
Premier League sides Huddersfield, Brighton and Cardiff all went out, as did Championship high flyers Leeds and Aston Villa.
Macclesfield, Burton and Oxford march on to the third round.

West Ham manager Manuel Pellegrini speaking to Sky Sports: "Yes, I was worried because you play against these teams that defend really well and then you are losing 1-0 in the first minutes, after working the whole week not to suffer those kinds of goals.
"The most important thing was our application to win this game. I was sure if we continued playing that we would score those goals.
"The best thing the team did tonight was we kept calm, we continued moving and creating space.
"It's important to win. If we lose this game, that's another consequence - it's very important. We need to continue working and I hope on Saturday we can have our first victory in the Premier League."

AFC Wimbledon boss Neil Ardley speaking to Sky Sports: "I thought, if we kept two up front, we would've caused more problems and we felt with crosses into the box we could cause them problems."
On the red card: "At the time I was stunned, the first yellow is fair enough as he misses the ball. The second I think he has gone down very easy. In our league that happens all over the pitch and it's not given.
"It's a massive decision, we can't cause them problems after that and it was going to be attack against defence. It's a game changer."
On what Manuel Pellegrini said to him after the game: "He said your team was fantastic, they deserve a lot of credit and they made it very difficult for us."

One of the best own goals we will ever see.
Stoke clear the ball from their own box and Huddersfield's Juninho Bacuna attempts to volley it back into the opposition area.
Instead, he miscues it and from 30 yards out, lobs his own goalkeeper and the ball rolls into the net.
We'll be seeing that one again for a long time.

An almighty scramble in the West Brom box and Mansfield goalkeeper Bobby Olejnik almost scores but his shot is blocked and Krystian Pearce somehow rolls wide from a couple of yards out.
