That was a strange reaction from Andy Murray.

He bent over double and clutched his left hip and then his left thigh. Fognini is definitely giving that hip a workout.

It is getting fiery. Fognini went to argue his case with the umpire after being penalised, but to no avail.

He then faces a battle to hold on to his serve. A fortunate net cord from Murray leaves Fognini face down on the floor as he dived to get the ball over the net, unsuccessfully.

But Fognini recovers his composure, for a moment at least, with a fine overhead smash. It goes to deuce, but Fognini is in the right place at the net to send in a winning volley. Murray nets soon after and the Italian holds a lively service game.

I think he did a gesture with his finger. Something is brewing here. I picked it up but I am not going to say it.

Andy Murray is the world number one, but just produced a serve that gives hopes to players everywhere. It was so wide it even missed the doubles lines.

That is his fifth double fault of the match, and a sign that he is still a bit anxious about this match.

It does not matter though as he sends Fognini running around the court for 40-30.

However, the umpire then penalises Fognini for a "visible obscenity" and that point hands Murray the game.

Fognini has to dig deep in the well here mentally. He is sprinting well during the point and then limping over to the chair.

Fognini is still not running at full pelt, but his mobility seems to have improved a bit compared to earlier.

He is able to jog around the court and holds his serve thanks to a gentle touch with a beautifully-placed drop shot.

Over on court 18, Andy's brother Jamie is in doubles action with Bruno Soares. The pair have just gone into a fifth set decider.

Here's Andy and Jamie's dad catching a Jamie winner earlier in the day... BBC Sport journalist Chris Bevan is the bloke in the green t-shirt who almost gets Sam Groth's racquet in the chops.

Murray's first service game of the fourth set and it does not last long. A service game to love, in fact.

'Come on Andy, we are right behind you', shouts someone from the crowd, clearly happy with the Brit's efforts in that game.

Things are going Andy Murray's way on Centre but big brother Jamie and partner Bruno Soares are having a bit of a struggle over on court 18.

Sam Groth and Robert Lindstedt have levelled the match at two-sets all and are in confident mood judging from their banter with their coaches and other halves sat in front of me.

Fognini didn't register an ace in that third set, but comes up with one in the opening game of the fourth set. That helps him hold his serve.

Fognini has looked disinterested in the first set, fantastic in the second, injured in the third, now what in the fourth?

As you would expect in a set that ends 6-1, the stats were very much in Murray's favour, as you can see here.

Murray has stepped up the pace, aiming to capitalise on Fognini's misfortune with his ankle injury.

The Italian can bring it back to 30-30, before Fognini dumps a return into the net and gets into an argument with the umpire.

Seconds later another return is sent into the net and Murray leads two sets to one.

How much is that strapping on his ankle restricting Fognini? He hardly moves in that sixth game and it will be one of the easiest break of serves that Murray will have in this year's competition.

He breaks again, leads 5-1 and is serving for the third set.

Fognini looks to have something wrong with the Achilles, the back area of his ankle. He relies on his speed, he has some excellent wheels, but at the moment he looks to be in some big trouble.

Back on Centre Court, Fabio Fognini is getting some heavy strapping added to his right ankle.

'Let's go Andy, let's go' shout a couple of fans in the crowd, while the player himself goes through some practice shots as he aims to keep fresh while his opponent gets medical attention.

Will this injury ruin this match as a spectacle? We have written off Fognini once before and he proved us wrong then so you just never now.
