Young Sports Personality of the Year award nominations open

What have Andy Murray, Ellie Simmonds and Tom Daley got in common?

Not only have they each had an incredible 2012, but they have all won the prestigious BBC Young Sports Personality of the Year award.

Now it is time for someone new to have their name etched on to the trophy, but who will it be?

You have the chance to nominate your favourite young sports star of the year by simply downloading and completing a nomination form and returning it via email or post. All nominations must be received by Monday 19 November, 2012.

The award is made to the outstanding young sports person of the year, who was aged 16 or under on 1 January 2012, selected from nominations made to the BBC and by sports governing bodies via the Youth Sport Trust, and fulfil some other terms and conditions - which can be read here.

Either attach your completed form and email it to [e-mail] or print the form and post to: Young SPOTY 2012, Fourth Floor, Quay House, Media City UK, Salford, M50 2QH.

A panel of judges, including BBC television presenters John Inverdale, Ore Oduba and Sonali Shah, will finalise a shortlist of 10 and then a top three from the nominations received. The final three will then be invited to attend the live BBC Sports Personality of the Year 2012 show.

In addition to Young Sports Personality of the Year, there will also be awards for: BBC Sports Personality of the Year, Team of the Year, Coach of the Year, Overseas Sports Personality of the Year, Lifetime Achievement, Helen Rollason Award and BBC Sports Unsung Hero Award.
