US Kunduz hospital attack was 'avoidable human error'

Medecins Sans Frontieres (MSF) says an American military investigation into the attack last month by US forces on a hospital in Afghanistan has revealed a frightening catalogue of errors.

MSF accuses the military of gross negligence and violating the rules of war. At least 30 people were killed in the attack in Kunduz.

The US commander in Afghanistan has blamed it on "avoidable human error, compounded by process and equipment failures".
