  1. Prisoners with mental health problems 'not cared for properly once released'published at 13:31 British Summer Time 14 June 2017

    There is still not enough done to ensure prisoners with mental health problems are cared for after they are released, the review found.

    Nick Moor, from Niche Health and Social Care Consulting, which carried out the report, said: "The largest concern shared by all on the investigation panel and many of the people we interviewed was that despite these efforts, the challenges to ensure appropriate after care on release for prisoners with mental health problems remain.

    "If we were able to make just one recommendation to improve services and reduce the likelihood of such a tragic case happening again, it would be that the national services concerned (Department of Health, NHS England and Ministry of Justice) strenuously work together to improve the care and aftercare of prisoners with mental health problems."

    Among the report's recommendations are:

    *To improve the discharge planning of vulnerable prisoners with mental health problems who are released earlier than planned, including the development of clear guidelines for staff to refer to other mental health services

    *To improve information sharing between health and criminal justice services

    *To ensure the current pathway for released prisoners ensures those in need have access to mental health services upon release

    Dr David Levy, regional medical director at NHS England said Simelane was "let down by the NHS and other organisations involved in his care and treatment".

    "Treating patients is seldom done by one organisation, the health and social care system responds to the needs of individual people and their families," he said.

    "Sometimes, and in this case, the system gets it wrong and the consequences of when this happens can be devastating.

    "This is not acceptable and I unreservedly apologise to the families of both victims on behalf of NHS England and the organisations involved in delivering care to P before this incident took place."

    The family of Phillip Simelane is calling for people in the care system to be held personally accountable for the failures leading to the death of Christina Edkins.

    Simelane was jailed indefinitely after pleading guilty to her manslaughter.

    The family has called on the government to take immediate action and change procedures to ensure there is no repeat of the tragedy.

    The report commissioned by NHS England says Simelane's mother had reported his mental health problems, which included a number of attacks on her while at home and fearing for her life.

    The family of Christina Edkins say they have been left with a "never-ending heartache" after the 16-year-old was stabbed to death on a bus in Birmingham.

    The family have responded to the independent report into Phillip Simelane, who admitted the girl's manslaughter after years of mental health problems.

    "What makes it even harder for us is hearing and reading continuously that 'lessons need to be learned' whilst knowing that in reality very few lessons have been learned and, over time, little has actually changed," they say in a statement.

    The mother of a man who stabbed a schoolgirl to death made numerous calls reporting her concerns about his mental health.

    Philip Simelane's mother called health officials from 2004 when he was first assessed in a psychiatric hospital after he had taken an overdose.

    He stabbed Christina in a random attack on a bus in Birmingham in 2013.

    An independent review has now been completed investigating the care of the killer who had been treated for mental health problems one year before.

    Panel chair Ms Koran Bhogal, who carried out the investigation on behlf of NHS England, said the absence of access to partner organisations IT systems, information sharing, joined up thinking and working practice were all identified as failings in this case.

    The man who stabbed teenager Christina Edkins to death on a bus was deemed a risk but not to the wider public, a report has found.

    The 16-year-old was killed by Phillips Simelane as she travelled to school in Birmingham in March 2013.

    Simelane, from Walsall, was detained indefinitely under the Mental Health Act after admitting to her manslaugher.

    An independent review into the death of teenager Christina Edkins follows a report in 2014 which said the girls' death could have been prevented.

    Simelane, who was diagnosed with paranoid schizophrenia, was first identified a risk in 2004 when he was at school.

Based on these, and the escalation of his problems and offences, in our view it was predictable that P was at significant risk of reoffending, and that this would involve an act of violence.     The cause of a fire at an abandoned industrial site in Lichfield, which saw around 500 tonnes of waste ablaze, is being investigated. , external

    Staffordshire Fire and Rescue Service says, at its height last night, about 65 firefighters were tackling the blaze at the former GKN warehouse at the Crossfield Industrial Estate.

    They say the building had about 5,000 tonnes of baled waste inside and around 10% was on fire when they arrived after 19:22.

    West Midlands Ambulance Service, who were also called, say it was under control by 01:00 and fire crews are still at the scene this morning.

    Unemployment in the West Midlands has risen, latest figures from the Office for National Statistics show, external, despite a fall nationally.

    Between February and April, there were 6,000 more unemployed people in the region than November to January, giving a total of 166,000.

    Across the UK, for the same period, there were 50,000 fewer people out of work with 1.53 million unemployed.

    The West Midlands figure is also higher than the year before, with an increase of 6,000 compared with February to April 2016.

    There are "systemic faults" in Dudley Council's charging policy for care homes, an investigation has found.

    It comes after a family complained that they were given no choice but to pay an unaffordable top-up fee for their mother's care.

    The Local Government Ombudsman found, external the council had not followed statutory guidance.

    The authority will now refund more than £4,000 and agreed to review its procedures.

    De Montfort University has confirmed it is considering whether to strip disgraced BBC broadcaster Tony Wadsworth of an honorary degree given to him five years ago,

    Wadsworth and his wife Julie were jailed for five years on Friday after being found guilty of a string of indecent assaults on under-age boys.

    A spokesperson for the university said: "Following news of the verdict in this case, the university is now reviewing its award of an honorary degree made in 2012, in accordance with its procedures."

    Fire crews have spent most of the night fighting a large blaze at an industrial estate in Lichfield. , external

    Staffordshire Fire Service says crews were called yesterday at 19:22 to Crossfield Industrial Estate, off the A5127 Burton Road.

    They say the fire involved a lot of baled waste and they had to fight to stop it spreading to a "significant part" of the building - several surrounding roads were shut by Staffordshire Police.

    West Midlands Ambulance Service says they were also called, saying a large plume of smoke crossed the A38 and a nearby railway line, external.

    They say the blaze was under control by around 01:00, external.
