    Ms Martin, 29, was struck by a falling lid from a rotten water tank measuring approximately 1.5m (5ft) square, the inquest at Oldbury heard yesterday.

    The rotten timber fell six storeys from the Wolverhampton Mander Centre during the high winds in February.

    The inquest is expected to last five days.

    Kingsley Burrell, who was 29, died from cardiac arrest after being arrested following a disturbance in Birmingham in 2011.

    Constables Paul Adey, Mark Fannon, and Paul Greenfield were found not guilty of lying about his detention in police custody.

    A passenger train travelling at 125mph missed a group of line-side rail workers in Cheshire by "half a second", a rail accident report says.

    The incident happened at about 17:00 as the three workers crossed the tracks on the Dutton Viaduct between Crewe and Runcorn on 18 September.

    They managed to move clear "less than half a second" before the high speed train passed them at 125mph (200 km/h), according to the Rail Accident Investigation Branch (RAIB).

    No-one was injured, although the train driver was left shaken.

    The RAIB has carried out a preliminary investigation and is expected to publish a safety digest shortly.

    Two people have been taken to hospital after a two-car crash on the M54 this morning, the ambulance service say.

    A woman in her 30s, who was the driver of one of the cars, suffered a cut to the head and a minor leg injury - a man in his 20s, who was a rear-seat passenger in the other car is also being treated for a leg injury.

    Mrs Justice Carr told Aaron Barley, sentenced for the murder of Tracey and Pierce Wilkinson and the attempted murder of Tracey's husband Peter, he "may never be released" from prison.

    She told Barley he had shown "only regret that Mr Wilkinson survived his injuries and at times satisfaction in what you did achieve.

    "You knew that you were destroying the family. It is what you intended."

    Sentencing, Mrs Justice Carr told 24-year-old Aaron Barley: "The Wilkinson family, and in particular Mrs Wilkinson, had done nothing but their best to help you.

    "Mr and Mrs Wilkinson were both financially and emotionally generous to you.

    "You betrayed their trust in every way, exploiting the knowledge of their household that you had gained through their kindness to perpetrate your crimes.

    "It is difficult to imagine what went through Mrs Wilkinson and Pierce's minds when you attacked them, they must have been bewildered and terrified. One of them at least must have been aware of the stabbing of the other."

    The mother and son were stabbed in a "frenzied" attack at their home in Stourbridge in March.

    Mrs Wilkinson's husband was also stabbed six times by Barley, but survived after 11 days in hospital.

    Barley, 24, was taken in by the family who arranged accommodation, food, and a job for him after Mrs Wilkinson saw him trying to keep warm in a cardboard box whilst she was out shopping.

    The couple's daughter Lydia, 19, was away at Bristol University.

    West Midlands Police seized 11,550 uninsured cars from the region's roads over the past 12 months.

    The force says it doesn't have the resources to investigate a vehicle every time it flags up on an automatic number plate recognition camera, but officers have the power to seize and destroy vehicles they do stop.
