The incident happened yesterday evening at about 20:45 outside New Cross Hospital in Wolverhampton.
The cyclists were found "some distance from their bikes", said the ambulance service.
A woman in her 40s suffered suspected fractures to an arm and a leg and a man, in his 50s, had multiple lacerations and bruising to his legs.
“The driver of the car was not seen by ambulance staff", a spokesperson said.

A baton relay to celebrate 100 years of the RAF heads through Shropshire today.
The specially designed baton is visiting 100 sites associated with the RAF in 100 days, ending at Horse Guards Parade on 10 July.
Today, the baton is going from Shawbury on a bicylcle to Atcham, then by canoe to Jackfield and then by bike to RAF Cosford.

It was owned by the first actor to play the role, William Hartnell, and features his hand-written notes.
The episode was first broadcast in 1963, introducing William Hartnell as the Doctor.
The seller's grandfather discovered the script whilst refurbishing William Hartnell's flat, said Aston's Auctioneers.

The 21-year-old from Wantage, Telford's been charged with criminal damage to a motor vehicle and will appear before magistrates at a date yet to be set.

The arrest of a 20-year-old on suspicion of committing terrorism offences follows the examination of electronic items and documents seized from an address in Brookside Avenue, Coventry last week, police said.
The property was subject to extensive searches for several days by bomb disposal officers, and eight homes were evacuated.
A controlled disposal of a substance on scrap land was also carried out.
Ch Supt Mike O’Hara, from Coventry Police, said: "This is a complex inquiry that remains subject of a thorough investigation. The searches and evacuations were essential for officers and EOD to enable them to conduct their work and keep local people safe."
A man from Coventry has been arrested on suspicion of committing terrorism offences, police say.
The 20-year-old had previously been detained under the Explosives Act last Tuesday.
