  1. Water firms failed customers during 'Beast from the East'published at 07:44 British Summer Time 19 June 2018

    Some of the UK's biggest water companies, including Severn Trent, did not support customers properly during this winter's Beast from the East snow chaos, causing "significant hardship", Ofwat has said.

    Coventry carmaker Jaguar Land Rover agreed to stop production to preserve the water supply for homes, hospitals and schools and Cadbury also sent workers home during the freezing weather in March.

    The regulator said more than 200,000 customers were left without water for more than four hours and tens of thousands were cut off for days.

    Severn Trent, South East, Southern and Thames Water performed worst, it found.

    The Health and Safety Executive has been informed after a man died following the collapse of a wall at an address in Rugby where building work was taking place.

    Emergency services were called to Rugby Lane, Stretton-on-Dunsmore, Warwickshire, at 15:50 on Monday.

    The man, in his late 40s, was taken to hospital by air ambulance and later died.

  3. Lethal weapons surrendered during amnestypublished at 07:21 British Summer Time 19 June 2018

    More than 100 weapons have been handed in to West Midlands Police during the latest firearms surrender.

    The two-week campaign was extended to three after a spate of shootings in the region in May.

    More than 30 lethal weapons were handed in, along with ammunition, imitation firearms and dozens of air weapons.

    All those seized will be forensically tested before being destroyed, said the force.

    A GP surgery has been put in special measures after being rated inadequate by health inspectors. , external

    Marshall Street Surgery, in Smethwick, was criticised in three areas out of five by the Care Quality Commission (CQC).

    Inspectors said both effectiveness and management were inadequate, while in the safety category the surgery required improvement.

    The CQC has now put the practice in special measures, allowing it six months to show progress and improvements.

    Neither Marshall Street Surgery nor Sandwell Clinical Commissioning Group, which oversees primary care, have responded to requests for comments.

    Police were called just before 01:00 to the M50 near Pendock, Worcestershire, between J1 and J2.

    They say just the one vehicle was involved in the crash and the driver, in his 30s, was taken to hospital with what are thought to be serious pelvic injuries.

    They say his injuries aren't thought to be life-threatening.

    Wolves have confirmed the signing of goalkeeper Rui Patricio, external.

    The 30-year-old was part of the Portugal team which won Euro 2016 and has kept goal for the national team already in World Cup 2018.

    He was available to move to Molineux after joining six other Sporting Lisbon players in terminating their own contracts earlier this month.

    The Portuguese Players' Union said the players made the move in response to an attack by fans.

    The 58-year-old from Newcastle-under-Lyme was riding on the A530 Middlewich Road near Nantwich when the collision happened, police say.

    The man was involved in a collision with a blue Mazda at the junction of Colleys Lane and died at the scene.

    Due to be heard at a Strategic Policing and Crime Board meeting on Tuesday, the report prepared for Labour PCC David Jamieson, also outlines how West Midlands Police harbours serious concerns about the government’s "apparent lack" of contingency planning for a no-deal scenario.

    The report says the UK "risks falling off a cliff-edge in terms of certain security and UK-EU policing cooperation" and highlights investigations related to terrorism, child sexual exploitation and organised crime.

    The BBC has contacted the Home Office for a response, but Theresa May has previously called for a "special partnership" with the EU on security after Brexit.
