Rachel Johnston died two weeks after all her teeth were taken out at a dental service run by Worcestershire Health and Care NHS Trust, criticised for its drastic full extractions.

The NHS Trust in Worcestershire insisted it followed the correct procedures for vulnerable patients, while three clinical commissioning groups are investigating Ms Johnston's death.
