A family in Walsall who were mistakenly given used Covid test swabs have been given a personal apology by the councillor responsible for health.
Stephen Craddock visited Tracy Patel and Andy Witton yesterday.
Walsall Council has also said practices have changed, to ensure mistakes won't happen again.
The error occurred when a resident put the sealed used tests back in the grey bags they arrived in, instead of the box and handed them to a distributor, the Local Democracy Reporting Service said.
Residents are now being urged to put used swabs in the boxes provided and seal them up before waiting to be collected, instead of leaving their homes to hand them to distributors.

Birmingham Airport will receive a loan of up to £18.5m from the city council and might need more in the future.
Passenger numbers between April and December last year were down 91% because of coronavirus and members of the authority's cabinet agreed to the loan at a meeting yesterday.
The meeting heard all seven local authorities in the West Midlands were asked for help and four are contributing to the loan along with a shareholder, Ontario Teachers' Pension Plan.
But Conservative councillor Meirion Jenkins said: "Right now, looking at the prospects for air travel, I really don’t know what the future holds.
"My question is – if we put this money in… how confident are we that more won't be required?"
Council leader Ian Ward said he is "reasonably confident this is a number that we will not have to revisit."
But added: "We don’t know with any certainty what will happen with the pandemic so there is always a possibility that this sum of money will not be sufficient."
At Prime Minister's Questions this lunchtime the Birmingham Northfield MP has told the House of Commons thousands of people in his constituency have been having tests because of the emergence of the South African Covid-variant.

The company, which owns Bulmers, said it is cutting 10% of its global workforce, meaning 8,000 people will lose their jobs at its sites around the world.
But after cutting fewer than 100 jobs in the UK at the end of last year, it said the process was complete in this country, with most of those jobs going at Heineken's London head office.
The company said its yearly profits had gone down by almost 50% to £1bn during the pandemic.

Read MoreThe cost of making changes to English maternity units in the wake of the Shropshire baby deaths scandal could cost up to £400m a year.
That figure has been put forward by NHS Providers, an organisation representing NHS trusts, and it said at least £250m would be needed annually to achieve the required increase in workforce numbers.
The Ockenden Review into baby deaths at the Shewsbury and Telford Hospital Trust said maternity staff had caused distress to patients by using "inappropriate language" and blaming grieving mothers for their loss.
It identified seven "immediate and essential actions" needed to improve maternity services in England.
Chris Hopson, chief executive of NHS Providers told The Independent: "Trust chief executives feel strongly that this is a really important agenda but they can't implement the recommendations from the Ockenden Review in full unless there is more funding made available."
