A police drone pilot shouted: "Look out!" to warn people after a drone crashed in a street.
The crash happened during a training exercise above Maidstone Road, Leicester, on 3 March 2019 at 11:25.
The drone was flying about 300 ft (90m) but, after five minutes, it began to spin out of control and hit the ground.
Nobody was injured but the drone was instantly destroyed.
The Air Accidents Investigation Branch, which has just published its report into the crash, said examination of the recorded data showed that the rear left motor had failed.
In a statement, Leicestershire Police said: "Drones of the same make and model were grounded by the force as a safety precaution."
It added the drone was being flown within the manufacturer's guidelines.

A three-storey cannabis farm with more than 600 plants has been found at a house in Alfreton.
Police had been looking for two men suspected of being involved in a fight when they made the discovery.
The estimated value of the plants is thought to be in excess of £450,000.
Police had been responding to a call about a disturbance on Mansfield Road in the early hours of Tuesday.
After arresting three men, officers went looking for two others that fled the scene.
Members of the public directed them to a house where they might be but, instead of finding the men, they found a cannabis farm.
The three men, who were arrested on suspicion of public order offences, were found to be Albanian nationals who were in the country illegally.
Derbyshire Police said they have been passed on to officers from Immigration Enforcement for deportation back to Albania.
