Judge Simon Drew QC told running champ Lauren Jeska her stadium triple stabbing was a "cool, calculated" attack carried out with "chilling precision".
Jeska had been in dispute with UK Athletics over issues connected to her transgender status and, the prosecution told Birmingham Crown Court, she'd had a meeting to try to resolve it with the body's employee and one of her victims Ralph Knibbs - a week before the attack in March last year.
But the judge said meetings had not resolved to her satisfaction, adding the case was not about the dispute but "what happened thereafter" at Birmingham's Alexander Stadium, included the attempted murder of Mr Knibbs, a former player for Bristol RFC, to which she had earlier admitted.
Jeska, 42, of Machynlleth, Powys, has today been sentenced to 18 years in prison, with the judge having told her "it's clear that you have had many issues to struggle with" and "one came to a head last year".
Defence counsel Julie Warburton, in mitigation, said the dispute - which had caused a mental breakdown - was over hormones, with Jeska fearing she would have to undergo tests that would bring into public her transgender status - against, Ms Warburton said, a backdrop of whether her race results would be expunged.
Two other UK Athletics employees were injured when they intervened as Jeska set upon Mr Knibbs as if, the prosecution said, she was "trying to skewer meat".
Running champ Lauren Jeska has today been sentenced to 18 years in prison for stabbing three people at an athletics stadium.
Julie Warburton, mitigating, said Jeska's transgender status led to a disagreement with governing body UK Athletics of which three employees were hurt at Alexander Stadium in Birmingham, leaving one - Ralph Knibbs, a former Bristol RFC player - with life-threatening injuries.
Jeska, 42, of Machynlleth, Powys, was England's champion fell-runner three times between 2010-12 and had been a member of Todmorden Harriers, Aberystwyth Athletics Club and another club based in Snowdonia.
Problems arose when she was considered for international selection, Ms Warburton said.
Although she was not selected "she had a mental breakdown", Ms Warburton said, sparked by a dispute over her hormones.
Jeska was told her previous running results could be expunged and she feared further blood tests would reveal her transgender status publicly.
The main victim of runner Lauren Jeska's attack on top athletics staff at Birmingham's Alexander Stadium was Ralph Knibbs who was stabbed, the court heard, as if she was "trying to skewer meat".
Richard Atkins QC, prosecuting, said blood was pumping out of Mr Knibbs' neck after being stabbed several times.
He sustained a 2cm hole in his neck and was left with life-threatening injuries.
He also suffered a stroke at the scene which caused loss of sight.
In a victim impact statement, Mr Knibbs - former Bristol RFC centre and head of human resources and welfare at UK Athletics - told the court nerve damage had left him with difficulty eating. He said he was also still in pain following the attack by Jeska, 42, of Machynlleth, Powys, and had limited vision in both eyes.

A Lib-Dem mayoral candidate was mistakenly billed in campaign material as standing for Labour.
Households were sent a leaflet giving "three reasons to back Beverley Nielsen for West Midlands Labour mayor".
Quote MessageThere are a great many volunteers working hard with local Lib Dem campaigns at the moment, especially with the mayoral election coming up. The leaflet was produced locally by the Hall Green group, not by my office, and I'm very grateful for their support. I'm sure they were as shocked as I was when the mistake was pointed out."
