    Inspections carried out by the Care Quality Commission have found two surgeries in Coventry to be "inadequate".

    Cheylesmore surgery, external in Quinton Park and Willenhall Primary Care Centre, external were both inspected in September last year and found to be putting patients at risk because "systems and processes were not in place to keep them safe".

    Both now have six months in which to address the issues.

    They say it's hoped the document will be published by summer. 

    Warwickshire Police and Crime Commissioner Philip Seccombe said more use of preventative measures such as gates and barriers would be looked at.

    Rescue hound Chip has finally been re-homed after spending more than 1,000 days at the Dogs Trust in Kenilworth - its lodger of the longest standing.

    It says Chip is to live in Banbury with his new owner who said it was a case of "puppy love at first sight".

    Most dogs stay at the centre for an average of 39 days before being re-homed.

    A former prisoner from Coventry has spoken out about conditions inside HMP Hewell in Worcestershire, which he claims is "rife with drugs".

    Adrian Mason was on remand at the jail last year. 

    Earlier this week the jail was criticised in an inspection report for its high levels of violence.

    In a statement, the Ministry of Justice said a major shake-up would see "2,500 extra prisons officers and new security measures to tackle drones, phones and drugs and help make prisons places of safety and reform".

    Two tonnes of sand is being used to clear a diesel spillage on the M42, says Highways England.

    Two lanes remain closed southbound between J9, M6 Toll (Curdworth) and J8, M6.
