A Serious Case Review out today shows there were several missed opportunities by social services, health services and police, to protect Poppy Widdison (pictured) from Grimsby before she died.

Three years ago Poppy was found at her home in Grimsby looking lifeless.

What caused her death is still not known, but tests showed that Poppy had ingested a cocktail of drugs for up to six months before her death - including heroin, sedatives, methadone and ketamine.

Last month Poppy's mother 37-year-old Michala Pyke and her ex-partner 40-year-old John Ritting were found guilty of child cruelty after feeding Poppy illicit drugs at Ritting's home in Grimsby.

Today's report criticises services designed to protect Poppy and help her mother with her drug use.

Despite the significant evidence known to various agencies, not one service or person knew what Poppy's life was like or the extent to which she was exposed to risk and harm on a daily basis.

Pyke and Ritting will be sentenced next week.

We'll find out next week how a safety review is going of a road in North East Lincolnshire where a family of five died.

Three years ago five members of the Cockburn family died in a crash on the A18 near Laceby just outside Grimsby.

The safety review into the road was commissioned after an inquest into their deaths and a series of inspections were carried out last August.

The stretch of the road under review is between the junction with the A46 Laceby roundabout and the county boundary to the south.

Next week cabinet members will consider the independent report into the feasibility of bringing that stretch of the A18 to the standard of a modern A road and also how the road could be made safer.

  12. Missed opportunities to protect Poppypublished at 14:27 Greenwich Mean Time 11 January 2017

    Laura Foster
    BBC Look North

North East Lincolnshire Council has admitted that it missed opportunities to protect a four-year-old girl who died after being forced to take drugs by her mother.

Poppy Widdison collapsed at her home in Grimsby in June 2013.

Last month her mother and ex partner were found guilty of child cruelty after feeding her sedatives because they considered her to be an incovenience to the relationship.

Now a review has found that not enough people knew about the state in which Poppy was living.

North East Lincolnshire council says it fully accepts all the findings and says that while lessons have been learned from this review, it argues that many aspects of practice and processes have changed since 2013 when Poppy died.

    BBC Weather

There is a yellow warning for snow in the region on Friday and Saturday.

A yellow warning means "be aware".

Snow is forecast from the early hours of Friday morning and further showers of snow, sleet and hail will continue across parts of the UK throughout the day.

There will also be strong winds with northwesterly gales and large waves will affect some eastern coastal areas, perhaps leading to local over-topping.

    Weather graphic

On Saturday showers of sleet and snow will continue.

Further accumulations of snow between two and four centimetres may affect lowland areas while high ground may see locally five to 10 centimetres deep..

Disruption to transport networks seems likely either due to snow or ice.
