Inspectors gave Willersley House near Hull the top mark, Outstanding in all five assessment categories.
The Care Quality Commission inspects around 22,000 care homes and Willersley House is one of just 8 across the country to get top marks across the board.

Official figures show recorded crime in Yorkshire and the Humber increased by 18% between September 2016 and September last year.
South Yorkshire saw a large rise in violent crime - up 62% with homicide rates increasing in North Yorkshire by 40%.
West Yorkshire saw a 29% rise in sexual offences and East yorkshire had a 40% increase in house burglaries.
Nationally the figures also show a rise, but the separate crime survey which is based on interviews around the country suggests crime has continued to fall.
Hull Trains says its working hard to keep cancellations to a minimum following complaints from customers using the service.
More than 20 services have been cancelled this month forcing passengers to take alternative trains or replacement buses.
Hull Trains says that over the last month they've been "operating a reduced fleet" due to "extensive repairs required on one of their trains" after an incident on 7 December.

Speaking via the internet, Kate says she's been booked onto two trains which have been cancelled since December.
North Lincolnshire Council is putting forward proposals to increase the amount of council tax unemployed people pay.
The full council will look at putting it up from 25% to 50%.

Hull City Council has taken the rare decision to pay a playwright to write a new show for them every year.
When Hull New Theatre re-opened after renovation last September, John Godber's play Kings of Hull was one of the first to be staged at the venue.
The company running the theatre for the council says the show was so successful, they've asked him to write a sequel.

One of Hull's biggest employers Reckitt Benckiser is mounting a bid to take control of part of the US Drug company Pfizer.
Pfizer, which makes Advil painkiller, Centrum multivitamins and Chapstick lip balm is reported to want to sell for around $220m so it can concentrate on prescription medicines.
GlaxoSmithKline are also said to be putting together a takeover bid.
The deal could make Reckitt Benckiser one of the biggest global players in consumer health.
East Yorkshire's Kyle Edmund said he 'had caught the bug' for competing in big Grand Slam matches after his Australian Open run ended in the semi-finals.
The 23-year-old from Tickton near Beverley was beaten 6-2 7-6 (7-4) 6-2 by Croatian sixth seed Marin Cilic in Melbourne.
Edmund refused to blame a hip injury that required treatment, preferring to focus on two weeks that saw him make his major semi-final debut.

An inquest has heard how a young woman died after losing control of her car, external before colliding with a tree.
Plans for a £170M energy from waste centre outside Immingham have been submitted to North East Lincolnshire Council.
Once built, North Beck Energy says the plant would create 40 full time jobs.

A special ceremony to honour and remember Holocaust victims is to be held in Hull this evening.
Holocaust Memorial Day is being marked with an event at the Guildhall, which will feature guest speakers, readings and music.
Residents and community groups are being encouraged to attend the service, which begins at 19:00 tonight.
John Hewitt, Lord Mayor of Hull, says: "25 January provides an opportunity for everyone to learn the lessons of the Holocaust and subsequent genocides that have occurred."

Routine surgery and some outpatients clinics are to be cancelled at two East Yorkshire hospitals to help relieve pressure on emergency care.
The Trust which runs Hull Royal Infirmary and Castle Hill hospital says More than 8,000 hospital workers are launching “Operation Wintergreen” next week to deal with an unprecedented amount of pressure.
Routine surgery and some outpatient clinics will be cancelled for eight days starting on Monday 29 January to free up clinical teams in specialisms including cardiology, rheumatology and respiratory illnesses.
Consultants and nurses will then be redeployed to work alongside front line teams in Hull Royal’s emergency department.

The sign and a bronze plaque had been taken down following souring relations between Hull City's owners and the council.
Recent talks between the two led to a joint statement being issued earlier this week, in which Hull City owner Assem Allam said as a 'gesture of goodwill' the sign and plaque would be restored.
