An investigation has been launched after wagons derailed after running two red signals in Nottinghamshire.
The Rail Accident Investigation Branch (RAIB) said the rake of 22 wagons - 21 of which were loaded - ran away from a siding at Old Bank in Toton and passed the signals before the front four derailed in the early hours of 17 January.
The wagons - which had an estimated weight of about 1,870 tonnes - ran away for a total distance of about half a mile, the RAIB added.
A RAIB spokesman said: "Our investigation will seek to identify the sequence of events which led to the accident.
"It will also consider the processes that were in place for securing trains within the sidings, any factors relevant to how the risk of runaway wagons from within the sidings was being managed, the performance and design of the trap points, and any relevant underlying factors."
Hospital bosses say the letters were sent due to a "processing error" and were "extremely sorry".

Some clinically extremely vulnerable children have mistakenly been sent letters inviting them to receive a coronavirus vaccination.
The processing error was made by Nottingham and Nottinghamshire Clinical Commissioning Group (CCG).
Currently no-one under 16 is being advised to have a Covid vaccine.
Earlier today, the CCG apologised after adults classed as clinically vulnerable were sent letters inviting them to book their vaccine with an incorrect link. It is thought to have affected about 18,000 people.
This appears to have been the result of the same processing error.
