  19. 'Our son's death could have been prevented'published at 11:58 British Summer Time 16 June 2017

    Charlotte Wright
    BBC Radio Lincolnshire

The parents of a Lincolnshire man who hanged himself after leaving a mental health unit say their son's death could have been prevented.

Graeme Mutton from Market Deeping died after being allowed unescorted leave by staff at Peter Hodgkinson Centre in Lincoln. His body was found at Greetwell Road quarry in Lincoln in November 2015.

Yesterday, the coroner at his inquest recorded a narrative verdict and said no formal risk assessment had been undertaken before he was given unescorted leave.

In a statement, his parents Alan and Sheila Mutton said there were several missed opportunities and that record keeping was inaccurate and there weren't enough staff.

    Quote Message

Graeme was a loving and caring son, brother, father and uncle and a dedicated professional who cared passionately about education and the young people in his charge. He is sorely missed by all his family.

Alan and Sheila Mutton, Graeme Mutton's parents

They also said that although nothing could bring him back they hoped steps would be taken to ensure other families didn't suffer in similar circumstances.

The Lincolnshire Partnership NHS Foundation Trust has apologised and says it's made changes to his procedures.
