"Orient had the best of that 15-minute period. This is so hard to call, both sides are going for it but neither can make it count in the final third."
The whistle goes and it is half time in extra time. I'm going to call this now, penalties all the way.
Alex Revell has played all he can and Steve Evans opts to take him off for Newcastle loanee Haris Vuckic. No hat-trick for Revell, but what a goal he has for his memories.
Extra time appears to be taking its toll on Rotherham striker Alex Revell, who is jogging a little gingerly, but he finds a bit of energy to dink a ball into the path of Kieran Agard, but Jamie Jones is alert to the danger and races out to scoop up the ball. At the other end, Nathan Clarke heads wide from a free-kick.
Leyton Orient are swarming all over Rotherham but the Millers defence is holding firm at the moment. David Mooney attempts to curl a cross towards the far post but it is a tired hit as the ball goes high and wide of goal.
Fantastic reaction save by Leyton Orient keeper Jamie Jones prevents Scott Cuthbert from scoring an own goal. A long throw is flicked on and Cuthbert's attempted clearing header is straight at his own goal but Jones bats the ball away instinctively. Just how vital could that prove?
The referee blows his whistle and a further 30 minutes of what has been a pulsating encounter gets under way.
BBC Sport's Rob Stevens at Wembley: "Both managers will be hoping to impart some wise words in the break before extra time.
"These moments could be crucial for both sets of players."

"It's all about keeping your nerve now. Both sets of players will be stressing the importance of not making any errors. Neither side will want it to go to penalties, but they won't want to make any mistakes either."
They finished the regular season on the same points and their two league games had an aggregate score of 2-2. There is genuinely nothing between these two sides. Not a single player has given less than their all in this game, with all involved desperate to be the one that helps haul their side into the Championship. Compulsive viewing and only in the play-offs.
That is it and we are going into extra time.
Leyton Orient striker David Mooney hits an effort into the legs of Kari Arnason and the ball deflects behind for a corner, which comes to nothing. It looks like we are going to have an extra 30 minutes.
The fourth official holds up the board and we are going to have FIVE minutes of added time. What chance of more late drama like we had in yesterday's play-off final?
Ben Pringle shrugs off that booking to almost set up a late winner for Rotherham, his long ball into the area finding Kieran Agard, but the striker can't get the right connection as the ball bounces off his shoulder. He then heads wide at the near post from Lee Frecklington's cross. Rotherham knocking on the door at the moment.

"We're into the last few minutes of normal time and you couldn't make a decision on the outcome. Both sides have had a good season and here we are - there's nothing to separate them."
Ben Pringle earns a booking after upending Shaun Batt, whose pace has already caused Rotherham one or two problems.
Stephen Nicol:, external That Alex Revell goal is why I love football, changed with one piece of sublime skill. Incredible.
The stats favour the Millers at the moment, with Rotherham having had eight shots on target to Orient's two, which both resulted in goals. Kieran Agard has the chance to add to Rotherham's shots on target count, but fires high and wide. Just seven minutes left before this game goes to extra time.
Robert Milsom's first action is to almost let Leyton Orient take the lead as his poor clearance is straight to the feet of Lloyd James, who sends a deep cross from the right towards the far post but the ball just evades the reach of David Mooney. A let-off for the Millers.

"I'm surprised Leyton Orient have taken Dean Cox off. He gives the team something, he can be creative and he has scored. Russell Slade might be thinking that his side is being out powered. We're in for an enthralling end to this match. The opening 25 minutes were edgy and both teams were testing each other out, but it's been a great second half."
