Matt Everard, 46, from Billericay, Essex, set a speed of 74.306mph (119.583kmh) at Elvington Airfield near York.

PC David FieldsImage source, South Yorkshire Police

PC David Fields died when his BMW patrol car hit a vehicle on the opposite side of the road on the A57 on Christmas Day. The passenger of the other car, Lorraine Stephenson, 61, also died in the crash.

A report by the Independent Office for Police Conduct (IOPC), published following an inquest into PC Fields' death concluded, found the officer was driving at speeds of up to 123mph prior to the collision and had "temporarily lost control of his car" on a roundabout shortly before the fatal crash.

Lorraine StephensonImage source, South Yorkshire Police

IOPC Regional Director Miranda Biddle said: "It is evident that PC Fields made a series of errors that evening; he did not adjust his driving for the poor weather conditions, continued driving at speed, and it ended in the loss of Mrs Stephenson’s life and his own."

"Our findings in this case are clear, dispassionate and objective, as they should be.

"But this does not mean we cannot be empathetic in understanding the tragic circumstances of this incident – not least because of when it happened – and the impact that this has had on all those affected.

  6. Town council leader defends £1,000 ceremonial robepublished at 16:16 British Summer Time 13 May 2019

    Local Democracy Reporting Service

The leader of Morley Town Council has defended a decision to spend up to £1,000 on a ceremonial robe for the town mayor, stating it helps promote a positive image of the town to potential employers.

Recently-published town council papers reveal the decision to commission the robe using money from the authority’s special projects budget.

One Morley councillor criticised the move, claiming it is “the wrong direction to take”, while government spending cuts for local authorities continue.

Despite council minutes suggesting the exact cost, of £1,010.22, the leader of the town council Robert Finnigan claims the amount would be considerably cheaper than this.

He said: “The cost at least a couple of hundred pounds less than that.

“It’s promoting Morley to the wider area, and if we are going to do that, we don’t just turn up in jeans and trainers – it supports the work that the Mayor does across Yorkshire."

  15. Hillsborough families react angrily to Mackrell sentencingpublished at 13:08 British Summer Time 13 May 2019

Families of the victims of the Hillsborough disaster have been reacting to the sentencing of former Sheffield Wednesday club secretary Graham Mackrell, who has been fined for failing to discharge a duty under the Health and Safety at Work Act in relation to the stadium's turnstiles.

    Louise BrookesImage source, EPA

Mackrell, who is the first person to be convicted of an offence relating to the tragedy, failed to ensure there were enough turnstiles to prevent large crowds building up.

He was fined £6,500 and ordered to pay prosecution costs of £5,000 at Preston Crown Court.

Speaking outside court, Louise Brookes, whose brother Andrew died at Hillsborough, said the sentence was "shameful", adding:

    Quote Message

Our 96 deserve better than this and us families deserve better than this. We are all getting on in age and enough is enough.

Christine Burke, whose father Henry died in the disaster, said:

    Quote Message

He was in charge of the safety certificate at the time and he should have been sacked straightaway. There should have been no way that he could have pursued his career as he did do after Hillsborough.

    Quote Message

He went on to bigger and better things, he was promoted. This is a man who has been paid very well and gone on to do other things. That should not have happened.

    Dave Guest
Chief reporter, BBC North West Tonight

The man responsible for safety at Hillsborough in 1989 should have realised the risk of too many spectators for too few turnstiles before kick off at the Liverpool v Nottingham Forest game, a judge has said.
