Joe Ward scored a superb equaliser for Derby
Josh Vickers saved the decisive penalty from Karlan Grant to send Derby past West Brom in a penalty shootout to reach the second round of the Carabao Cup.
Aune Heggebo's first goal for the Baggies gave them the lead midway through the second half and it looked like being enough to take them through.
But in the last of six minutes added on, Joe Ward whipped a free-kick from the left into the top-right corner to force penalties.
It was a superb finish but Derby's only shot on target in a game in which they were very much second best.
West Brom came close to an early lead when George Campbell rose highest to head down a free-kick and Heggebo was inches away from turning it in.
But it took until the 67th minute to break the deadlock. Callum Styles' floated pass was headed across goal by Isaac Price and into the path of Heggebo to score.
Moments later, Derby should have levelled when Ward crossed for Lennon Wheeldon, whose powerful header flew wide.
With four minutes remaining, Josh Maja could have won the tie with a shot from the edge of the area but it was parried away by Vickers and it proved to be a crucial save.
